What are the question types in the form builder?

Questionnaire:

In the questionnaire, various question types allow you to create questions from scratch. We provide options such as single choice, multiple choice, dropdown, single-line input, multiline input, linear scale, rating stars, media, file, yes/no, and eSignature. Each question type corresponds to its name.

  • Single Choice: Edit the question name, add a description, and create multiple options. Respondents can select only one option.
  • Multiple Choice: Similar to single choice but allows respondents to check multiple options.
  • Dropdown: Edit the question name, add a description, and create a list of options. Respondents can choose one option from the dropdown.
  • Single Line Input: Edit the question name, add a description, and choose the answer format (none, number, email, phone, url). The answer length is limited to 50 characters.
  • Multiline Input: Similar to single line input but allows longer responses (up to 5000 characters).
  • Linear Scale: Edit the question name, add a description, and provide a scale of numbers (1-10). Respondents choose one number, and you can customize the sorting order.
  • Rating Stars: Edit the question name and description. Respondents rate using a fixed 5-star scale.
  • Media: Edit the question name, add a description, and choose the type of media (e.g., images). Set a maximum limit for uploaded media (e.g., up to 3 images).
  • File: Edit the question name, add a description, and set the maximum number of files that can be uploaded (e.g., up to 5 files).
  • Yes/No: Edit the question name and description. Respondents can choose only between yes or no.
  • E-Signature: Edit the question name and description. Provides an area for respondents to electronically sign their names.

Check-In:

For check-ins, in the check-in builder, there are two categories, one is client info, the other is additional info, and for additional info, they are the same as those question types in questionnaire builder, they let you start from scratch. For client info, all the questions can’t be edited, they are all preset and there are three subcategory of client info questions,

Biometric(and if you want to sync. the  submitted check-in to client, then this type of questions will be synced to biometric body data of the client and can be found under client’s body tab)

  • Weight: Essentially it’s single line input question, with question name set to ask what’s your respondent weight.
  • Height: Essentially it’s single line input question, with question name set to ask what’s your respondent height.
  • Body fat percentage: Essentially it’s single line input question, with question name set to ask what’s your respondent body fat percentage.
  • Resting heart rate: Essentially it’s single line input question, with question name set to ask what’s your respondent resting heart rate.
  • Water rate: Essentially it’s single line input question, with question name set to ask what’s your respondent water rate.

Circumference (and if you want to sync. the submitted check-in to client, then this type of questions will be synced to circumference body data of the client and can be found under client’s body tab)
  • chest: Essentially it’s single line input question, with question name set to ask what’s your respondent chest.
  • waist: Essentially it’s single line input question, with question name set to ask what’s your respondent waist
  • neck: Essentially it’s single line input question, with question name set to ask what’s your respondent neck.
  • hips: Essentially it’s single line input question, with question name set to ask what’s your respondent hips.
  • arm: Essentially it’s single line input question, with question name set to ask what’s your respondent arm.
  • Thigh: Essentially it’s single line input question, with question name set to ask what’s your respondent thigh
  • Calf: Essentially it’s single line input question, with question name set to ask what’s your respondent calf

Journal (and if you want to sync. the  submitted check-in to client, then this type of questions will be synced to journal of the client and can be found under client’s journal tab)

  • Progress photo: Essentially it’s media question, with question set to let respondent upload images to track their progress. And the only media type can be uploaded is image and maximum number is 5. And also provided with a input box, to let respondent. write anything to note the photos.
  • Nutrition photo: Essentially it’s media question, with question set to let respondent upload meal images to track their nutrition. And the only media type can be uploaded is image and maximum number is 5.
  • Workout video: Essentially it’s media question, with question set to let respondent upload a video to record their workout to let you take this as a reference to check if the form is correct.  And the only media type can be uploaded is video and maximum number is 1.

Intake Form:

For intake forms, in the intake form builder, there are two categories, one is basic info, the other is additional info, and for additional info, they are the same as those question types in questionnaire builder, they let you start from scratch. For basic info, all the questions can’t be edited, they are all preset.

Basic Information
  • Gender: This is a single-choice question where respondents can choose from options like male, female, or others.
  • Birthday: This is a date picker question prompting respondents to input their birthdate.
  • Phone: This is a single-line input question, divided into two boxes—one for the country code and the other for the phone number.
  • Address: This question involves inputting the respondent's address.
  • Timezone: It is a dropdown question where respondents select their timezone from a list of global timezones.
  • Avatar: Respondents can upload their profile pictures.
  • Emergency Contact: Another single-line input question asking respondents about their calf preferences.
  • Fitness Goals: This is a multiple-choice question inquiring about fitness goals, with options such as Weight/Fat loss, Muscle building, Tone or get leaner, Overall health/fitness, Gain body confidence, Develop healthy habits, Knowledge around training & nutrition, Improve energy levels.
  • Training Preference: This set of questions includes a dropdown question about preferred frequency, a multiple-choice question about the preferred day of the week, and a dropdown question about the preferred time of day.

💡 Fitness goals answer options are customizable in the system settings under Settings/Client/Fitness Goals.

Preset Additional Information

  • Preferred Contact: This is a single-choice question asking about the preferred mode of contact—text message, voice memo, email, or phone call.
  • Activity Level: This is a single-choice question assessing the respondent's current activity level based on professional standards.
  • Obstacles: This is a multiple-choice question asking about the respondent's biggest struggle in achieving fitness goals, with options like accountability, consistency, time, knowledge, overwhelm, and motivation.
  • Training Experience: This is a single-choice question inquiring about the respondent's consistent training duration, with options ranging from never trained before to 5+ years.
  • Training Expectation: This is a multiple-choice question about the qualities respondents are looking for in a coach.
  • Training Commitment: This is a rating scale question allowing respondents to rate their commitment to achieving fitness goals.
  • Feel Free to Write: This is a multi-line input question inviting respondents to include any questions they have for the trainer.

Starting from Scratch (Additional Info)

This section mirrors the questionnaire builder, offering flexibility for users to create and customize additional questions as needed

Agreements:

For agreements, agreements content is pure text, there will not be any questions in the agreement builder. And Every agreement we will automatically add date, name input box to let respondent fill in, and e-signature to let your respondent sign your name.
